The Do Not Resuscitate is a document which informs emergency medical personnel and hospital emergency personnel to NOT resuscitate you. Also known as a Pre-Hospital Medical Care Directive.

You are asking emergency personnel to NOT use equipment, drugs, or devices to restart your heart or your breathing.

This document must be on letter-sized ORANGE-colored paper or wallet-sized ORANGE-colored paper to be valid and you must attach your picture or complete your personal information with your physical description.

This document must be signed by you in front of a witness and it must be signed by your witness and health care provider.
