Life Care Planning

Life Care Planning is the process of planning for future health care treatment decisions now. What this means is if at some point in the future you become incapacitated (not able to make known your opinions or ideas relating to health care treatment) someone you trust will be able to make those decisions for you. This person will act as “you” when you are unable to act on your own behalf.

The documents that allow for planning for future health care treatment decisions (also known as Advanced Directives) are:

1. Durable Health Care (Medical) Power of Attorney (DHCPOA)

2. Durable Mental Health Care Power of Attorney (DMHCPOA)

3. Living Will

4. Pre – Hospital Medical Directive also known as Do Not Resuscitate (DNR)

5. Advanced Directive for Oral Feeding and Fluids in Case of Dementia

6. HIPAA Waiver

7. Final Disposition

8. Last Will and Testament

Arizona Advanced Directives Registry

In Arizona, you can register your signed/notarized Durable Health Care (Medical) Power of Attorney (DHCPOA), Durable Mental Health Care Power of Attorney (DMHCPOA), and Living Will with the Arizona Advance Directives Registry.

Arizona Advanced Directives Registry is a database for the storage of advance directives (documents that plan for future health care treatment decisions now) and the Arizona Secretary of State oversees its security and operations. It allows health care professionals and family or friends you trust to act as “you” to view your Durable Health Care Power of Attorney, Durable Mental Health Care Power of Attorney, and Living Will. By allowing these people the opportunity to view these documents you are making them aware of your wishes even though you cannot express your wishes.

As a courtesy to you and your time, our firm has included both the Life Care Planning Packet from the Arizona Attorney General’s website and the Arizona Advance Directives Registry Registration Forms from the Arizona Secretary of State’s website here on our website.

When signing these documents please remember you will need to find a Notary Public or one unrelated witness to acknowledge the signing of your documents.
