Living Will

A living will is a document that allows you to express, in writing, which medical treatments you want or do not want at the end of your life or when you are in a vegetative state or in a coma.

It is a document that tells your doctor and your family how you want to be treated in these situations. In order for this document to be valid in the State of Arizona, this document must be witnessed by two people not related to you OR notarized.
